Concierge Primary Care Physician
Full-Time
Job Description
The Concierge Primary Care Physician owns a capped concierge primary care panel and is the physician around whom OneLife’s primary care offering is built. The role manages chronic disease, addresses acute issues, coordinates specialty care, conducts comprehensive annual evaluations, and maintains a standard of access consistent with concierge medicine.
The physician in this role also directs a supporting clinical team and shapes the clinical protocols the practice’s providers work from. OneLife’s model is collaborative by design. The clinical team presents and reviews cases together each morning, which builds daily education, patient continuity, and coordination between providers into the workflow rather than adding them onto it. Executing that process and developing it further is a central part of this position.
About OneLife
OneLife Health & Performance is a physician-led, membership-based medical practice in Palm Beach Gardens. We deliver precision concierge medicine alongside advanced diagnostics, regenerative medicine, hormone and peptide therapy, human performance, and aesthetics, entirely on a cash-pay basis. The practice has served communities in Florida for more than a decade and recently opened a 6,500 square foot flagship facility.
Advanced biomarkers, imaging, and analytics identify risk early, and care is directed at root cause rather than symptom management. OneLife presents itself to patients as their medical home, and this is the position that delivers on that commitment.
The practice is entering its next phase, with a new membership program, additional clinical services coming online, and new locations to follow. This position has been created to establish and lead concierge primary care at OneLife.
- Reports to: Chief Medical Officer
- Location: Palm Beach Gardens, FL · On site
- Type: Full-time · Exempt
Responsibilities
Panel and clinical care
- Own a capped concierge primary care panel, including chronic disease management, acute care, specialty coordination, and comprehensive annual evaluations.
- Maintain member access consistent with concierge standards.
- Integrate advanced diagnostics, biomarker panels, and hormone and metabolic optimization into primary care, not as separate services.
Team leadership
- Direct the clinical team supporting the panel, including physician assistants, nurse practitioners, nursing staff, health coaching, nutrition coaching, human performance coaching and patient care management.
- Lead daily case review. The clinical team meets each morning to present and review patient encounters, which is how the practice holds quality and develops its providers.
Division and protocol development
- Define the member experience, annual cadence, and clinical standards for concierge primary care.
- Contribute to the clinical protocols OneLife’s providers practice by, and to new service lines as they come online.
Scope and reporting
- Reports to the Chief Medical Officer.
- Direct reports: the clinical team assigned to the concierge primary care panel.
- Partners with the Chief Operating Officer on scheduling capacity, panel operations, and member experience standards.
- Partners with the Head of Marketing on membership growth and member retention.
- Clinical judgment, protocols, and provider practice remain with the physicians.
Requirements
Required
- MD or DO with board certification in Family Medicine or Internal Medicine, or board eligible with certification pending.
- Active Florida medical license, or eligibility and willingness to obtain one.
- Sound clinical judgment and demonstrated comfort managing complex primary care independently.
- Genuine interest in developing expertise in longevity medicine, metabolic health, hormone optimization, and advanced preventive care, whether or not formal training is already held.
- Discretion and professionalism appropriate to a high-profile patient population.
- Comfort practicing in an environment that is continuously being built and refined, where clinical protocols are shaped rather than inherited.
- Resident within reasonable commuting distance of Palm Beach Gardens, and on site in person during clinic hours each weekday.
Preferred
- Experience in concierge, direct primary care, or another cash-pay model.
- Training or certification in functional, integrative, or age management medicine.
- Prior work with peptides, bioidentical hormones, or advanced biomarker interpretation.
- Established ties to the Palm Beach, Jupiter, Palm Beach Gardens, Tequesta, and Juno Beach communities.
- Active or recently held privileges at a Palm Beach County hospital.
Additional Information
Growth and advancement
OneLife promotes from within. Depending on interest and performance, the path from this seat can include a medical director role as the practice expands. OneLife would rather develop a physician into those roles than recruit for them later.
Onboarding
Onboarding is structured and deliberate. The physician in this role arrives with established competence in primary care; what is new is the precision and longevity layer, and the Chief Medical Officer provides that instruction directly. Onboarding begins with shadowing across the clinical services, proceeds to independent consults, and concludes with full assumption of the panel. OneLife funds continuing education, certification, and conference attendance, and participation is an expectation of the role.
30 / 60 / 90 day expectations
First 30 days. Shadowing across the clinical services, EMR and systems training, a full review of the membership program and diagnostic evaluations, and direct instruction from the Chief Medical Officer in the practice’s longevity and precision protocols. Independent consults begin within this period. Panel enrollment has not yet started.
First 60 days. Panel enrollment begins. Daily case review is led rather than attended. A written assessment of the concierge primary care model is expected, covering what should be kept, what should change, and the first three changes the physician would make.
First 90 days. The panel is held outright, the access standard operates as defined, and a plan is presented for the following two quarters covering panel growth, clinical protocols, and readiness for the membership tiers this role gates.
Performance metrics
- Panel growth against plan, and panel retention through renewal
- Member satisfaction and referral volume from within the panel
- Response time measured against the defined access standard
- Clinical quality, including documentation, follow-through on diagnostic findings, and case review participation
- Conversion of diagnostic evaluation patients into membership
- Contribution to protocol development and provider education
Compensation and benefits
- Base salary. Competitive base salary, commensurate with experience.
- Performance bonus. A performance bonus structure applies in addition to base salary.
- Benefits. Health coverage, paid time off, malpractice coverage, and a retirement plan.
- Continuing education. CME budget with paid time to use it, including longevity and regenerative medicine training.
- Facility and support. A new flagship facility with advanced diagnostic and therapeutic technology, a capped panel, and a schedule built for full-length visits.
Confidentiality, HIPAA, and brand ownership
This role has access to protected health information, member financial records, pricing, and proprietary clinical material. Strict compliance with HIPAA and with the practice’s confidentiality policies is a condition of employment. All work product, protocols, documentation, and materials created in the course of this role remain the property of OneLife Health & Performance.
Performance evaluations
Performance is reviewed at 90 days and then on the practice’s annual review cycle. Compensation and bonus eligibility are assessed against the metrics above.
Definition of success
Success in this position is a concierge primary care division that runs on its own: a full panel that renews, members who refer their circle, an access standard that is met without burning out the physician who holds it, a clinical team that is measurably better for the daily case review, and membership tiers that opened on schedule because the medicine behind them was ready.
